Med Waiver Group Homes
We offer four (4) group homes for individuals aged 18+ who have been diagnosed with a developmental disability. Located in family neighborhoods in the Plantation and Fort Lauderdale area, each of these residences is home to six (6) individuals. Placement is based on similar disabilities and ages. Staff provide skilled nursing support, nutrition, and innovative therapy services to foster independence and daily living skills. Residents are encouraged to participate in local neighborhood activities to promote healthier outcomes and community inclusion.
Group Homes are individually tailored to meet the specific needs of the residents and consist of both single and double bedrooms. Residents are supported 24-7 by Resident Training Coordinators who provide supervision and assist with opportunities to actively participate in acquiring, improving, and maintaining skills related to activities of daily living, alongside social and adaptive skills that enable individuals to fully benefit from community living.
Each resident is intimately involved in their Person-Centered Planning process to ensure their individual wants and needs are met with a focus on self-determination and independence.
Optimal health and wellness is provided with 24-7 support from nutrition and nursing staff.

Must be 18+ years of age
An individual must have a developmental disability which occurs prior to age 18 and constitutes a substantial handicap that can reasonably be expected to continue indefinitely
Disabilities served include: Intellectual disabilities (full scale IQ of 70 or below), severe forms of autism, spina bifida, cerebral palsy, prader-willi syndrome, down syndrome
All potential admits must follow the APD application process
