Residential Care
Residential placement is a wonderful, inclusive and caring solution for families who can no longer meet the support needs of a loved one with a developmental disability.
Ann Storck herself recognized this need in 1951 and today we proudly offer individuals, families and guardians exceptional community living and integrated resident care options.
Intermediate Care Facilities (ICFs)
We operate two (2) Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID).
Fort Lauderdale ICF is a 48 bed facility and Pembroke Pines ICF offers 24 beds.
Each ICF has three (3) family-style homes supported by an around-the-clock team of residential staff, nurses and therapists who collectively provide intensive specialized services to individuals with developmental disabilities, behavioral challenges and/or significant functional skill impairments.
Each ICF is based on a medical and or behavioral model which provides all services (multidiscipline).
Placed individuals must have the ability to benefit from rehabilitative services, known as Active Treatment. Each individual is provided with an individual plan that is tailored to their specific needs. Their parent/guardian is a valued member of their Circle of Support.
Each ICF has a unique home-like setting. Bedrooms are individualized to likes and preferences. No two rooms are alike! Wrap around support services include 24 hour nursing and residential services, innovative therapy, recreation and community inclusion.
Eligibility For Placement

An individual must have a developmental disability which occurs prior to age 18 and constitutes a substantial handicap that can reasonably be expected to continue indefinitely

Disabilities served include: Intellectual disabilities (full scale IQ of 70 or below), severe forms of autism, spina bifida, cerebral palsy, prader-willi syndrome, down syndrome

Group Homes
We offer four (4) group homes for individuals aged 18+ who have been diagnosed with a developmental disability. Located in family neighborhoods in the Plantation and Fort Lauderdale area, each of these residences is home to six (6) individuals. Placement is based on similar disabilities and ages. Staff provide skilled nursing support, nutrition, and innovative therapy services to foster independence and daily living skills. Residents are encouraged to participate in local neighborhood activities to promote healthier outcomes and community inclusion.
Group Homes are individually tailored to meet the specific needs of the residents and consist of both single and double bedrooms. Residents are supported 24-7 by Resident Training Coordinators who provide supervision and assist with opportunities to actively participate in acquiring, improving, and maintaining skills related to activities of daily living, alongside social and adaptive skills that enable individuals to fully benefit from community living.
Each resident is intimately involved in their Person-Centered Planning process to ensure their individual wants and needs are met with a focus on self-determination and independence.
Optimal health and wellness is provided with 24-7 support from nutrition and nursing staff.
Behavior Analysis services are provided to residents who meet APD criteria. Funding is received through APD iBudget and potential applicants must receive Medicaid Waiver Services.

On-Demand Nursing Services
Nursing services are administered by an LPN (Licensed Practical Nurse). Delivery of nursing services is monitored by a Registered Nurse on site and is available 24 hours a day for consultation. Nursing services include participation by a Registered Nurse in the pre-admission evaluation, planning and placement of an individual. A full-time Registered Nurse Educator provides education to licensed staff and all direct support professionals. Nursing services include providing physician with assessments, participating in on site clinics, primary care, neurology, vision, gynecology; administration of all medications with reporting of adverse reactions; administration of all gastronomy feedings with replacement of feeding tubes at least every 3 months. For respiratory involved individuals: aerosol respiratory treatments, suctioning, oxygen therapy, BIPAP and CPAP therapy. Healthcare Progress Reports are documented monthly outlining healthcare goals, physician visits, illnesses and response to treatment.
Nursing Care is also provided to individuals residing in the Group Homes. Services include ensuring annual physician evaluations, consultation follow-up, monthly health reporting, and needed health assessments. All Group Home staff receive extensive training and in-servicing in medication administration, first aid and emergency care. The Registered Nurse Educator is also available to provide information and assistance with individual health concerns.
Licensed nursing services are also provided under the direction of a Registered Nurse to individuals who attend our daily programs (Adult Day Training and School). The nurse participates in the admission process and is responsible for medication administration, gastronomy tube feedings, respiratory care, first aid and emergency care, while assuring all services are provided with the proper authorization.
Additional nursing services may also be provided to meet individual health needs based on facility policies.

Integrated Nutrition
The Nutrition team is managed by a Registered and Licensed Dietitian to ensure quality adherence to state and federal guidelines. Our cooks are certified in Food Safety and Sanitation. Our kitchen prides itself on achieving 100% scores on all quarterly Broward County Health Department Inspections.
Serving three (3) meals daily for both ICFs that meet the most current requirements to ensure optimal nutritional health and well-being. Nutritious meals, snacks and beverages are also provided to participants of our daily programs. We individualize meals according to prescribed dietary and preparation needs such as puree, ground, chopped, and finely chopped.
The Nutrition team participates in Support Team meetings as an integral part of an individual’s total care. This multi-disciplinary approach is vital when planning, preparing, and presenting all aspects of nutrition to the individuals served. Monthly, quarterly, and annual Nutrition Assessments are provided. Each individual is closely monitored through review of records and observation at mealtime to recommend dietary changes for a variety of physical and medical conditions.
The team works closely with the other disciplines to attempt diet texture progressions and transition from feeding by tube to enjoying meals orally. They provide education and counseling to our individuals, their families, and caregivers to ensure the highest quality of dietary care.
